The site (www.szkolastomatologii.pl) works well, but after going to the subscription form appears:

Hello,

Well, i don't think it's directly an iCagenda issue...

First, try the form with Protostar site template, as yours is loading 2 times the jQuery library which could result in broken script on the page (so javascript form fields validation could be broken for this reason...)
